Role/remit

Provides high standard of health care to vulnerable and hard to reach groups both from a GP surgery base and an assertive outreach model.

Services for:

• Homeless/ single/ families
• Women experiencing/fleeing violence
• Gypsy travellers
• Refugees
• Migrant workers
• Prison leavers/people in the criminal justice system
• Looked after and accommodated children
• Young people
• Working with people in areas of deprivation
• Working within the health inequality agenda

Prevention work: primary

• Harm reduction and treatment of clients misusing or affected by substance misuse
• Testing and counselling for blood borne virus
• Prevention of homelessness
• Prevention of long term conditions through opportunistic contact and health check appointments
• Dental drop-in service
• Sexual Health
• Podiatry
